To request a copy of the marriage certificate, write a request note which must include applicants’ full names, applicants’ former names before marriage (if any), marriage date, and applicants’ return address. To obtain copies of birth or death certificate, start by completing a Birth Certificate Request Form or a Death Certificate Request Form. A copy of a valid and unexpired government-issued photo identification is required to process each application. The first copy of the birth certificate costs $26, while each additional copy requested at the same time attracts a $19 charge. The first copy of the death certificate is $13, while additional copies requested at the same time cost $6 each. Cash, credit card, and check payable to Scott County are accepted for payment.
